Incorporation Instructions & Forms
There are two forms which must be completed and submitted for Incorporation as a non-profit in the State of Pennsylvania, and a third form for receipt of an employer identification number for that new corporation. Detailed instructions for each form are included below. Sample completed forms are also available for each step. You should file the incorporation forms first. Once you receive a response back from the Department of State, you will have the date of incorporation needed to file for the employer identification number.

Instructions for completing the Articles of Incorporation
- Check “Domestic Nonprofit Corporation” at the top.
- Enter the name and address of the Rector. Do not use the street address of the current property if there is any chance that you will leave that location.
- Item 1. Enter the name of the parish. It is better to use the form “All Saints Anglican Church, Inc.” than the form “The Rector, Wardens and Vestry of All Saints Anglican Church, Inc.” The Corporation Bureau requires that you include “Inc.” That will be the official name that you put on legal documents and bank accounts, but you do not have to use it on informal material like stationary or signage.
- Item 2(a). An actual street address is required (not a P.O. Box) but it doesn’t have to be the actual location of the property. So if you are going to move but don’t yet have a permanent new address, you can use a home address.
- Item (b). Enter “N/A.”
- Item 3. You have flexibility here. You can say something as simple as “Worship of Almighty God” or you can be more expansive.
- Item 4. Leave as is.
- Item 5. Check “The corporation is organized on a non-stock basis.”
- Item 6. Strike out both statements.
- Item 7. Leave as is.
- Item 8. Enter the names and addresses of the incorporators. That can be one person or more. We have no recommendation.
- Item 9. Enter “Upon filing” unless you prefer a specific date – like the feast day of your patron saint.
- Item 10. Leave as is, and attach the page entitled “Additional Provisions” which follows.
- Signature block. Date and sign. All of those listed as incorporators must sign.
- Submit with a filing fee of $125 and a Docketing Statement to the Pennsylvania Department of State.

Instructions for completing the Docketing Statement
- In the box entitled “Pennsylvania Entities” check “nonprofit non-stock.”
- Item 1. Enter the name used in Item 1 of the Articles of Incorporation.
- Item 2. You probably want to use the name and address of the Rector.
- Item 3. Enter “Anglican Church.”
- Item 4. Enter the same thing you used for Item 9 of the Articles of Incorporation.
- Item 5. Enter “Applied for”
- Item 6. Enter “December 31” or whatever your fiscal year end may be.
- Item 7. Leave blank.

Instructions for completing the Application for EIN
- Item 1. Enter the name used in Item 1 of the Articles of Incorporation.
- Item 2. Enter “N/A.”
- Item 3. Enter “N/A.”
- Item 4a and b. Do not use the street address of the current property if there is any chance that you will leave that location.
- Item 5a and b. An actual street address is required (not a P.O. Box) but it doesn’t have to be the actual location of the property. So if you are going to move but don’t yet have a permanent new address, you can use a home address.
- Item 6. Enter county and state.
- Item 7a and b. Enter the name and social security number of the Rector.
- Item 8a. Check “No.”
- Item 8b and c. Leave blank.
- Item 9a. Check “Church or church-controlled organization.”
- Item 9b. Enter “PA.”
- Item 10. Check “Started new business” and enter “Church.”
- Item 11. Insert date of incorporation which will be stamped on the Articles of Incorporation returned to you by the Corporation Bureau.
- Item 12. Enter “December” or whatever your fiscal year end may be.
- Item 13. Enter “0” under “Agricultural” and “Household.” Enter the number of expected employees under “Other.”
- Item 14. Unless you expect to pay less than $4,000 per year in wages, leave this box blank.
- Item 15. Enter the date you expect to begin paying wages.
- Item 16. Check “Other” and enter “Church.”
- Item 17. You have flexibility here. Enter something like “Religious services to include worship, sacraments and other rites in the Anglican tradition.”
- Item 18. Check “No.”
- Third Party Designee. Leave blank unless some one else is acting as your representative.
- Type or print name. Sign and date. Provide telephone number and fax number where indicated.
